  1. Restrict Duplicate Diagnoses

    It would be great if the system would not let us add a diagnosis if it is already active on the patient's chart

  2. Diagnosis List Ranking & Linking

    It would be paramount to organization, flow and reimbursement if we could rank diagnoses in order of importance during each visit in the Assessment portion of the note, and then link diagnoses to procedure codes in the Plan of each note. Currently, I can rearrange the order of diagnoses, but it does not hold that order in Finalize or future notes. This is a quick and rudimentary method of justifying in each note, why each procedure was performed or how each diagnosis was addressed, without additional sentences or explanations that create clutter in a note.

    This is a solid idea. We’re currently digging into how our current Problem List works and this is valuable input for how we may shape this feature.

    How else would you like to see the Problem List evolve and improve?

  3. Phase out ICD-9 entirely

    We are now more than 6 months past the ICD-9-->ICD-10 transition and I don't need ICD-9 for my practice anymore. We would like for it to be removed from our daily workflow.

    Hello everyone – as of 12/5/17, ICD-9 codes have been removed from the diagnosis display in the patient Summary, encounter note, and superbill. Additionally, ICD-9 codes are hidden when viewing individual diagnosis details, though you can always expand the details to view mapped codes if needed. While you are still able to search by ICD-9 code, search results will return only the diagnosis description and ICD-10 code.

    Please note that this request is considered partially resolved as ICD-9 codes will continue to transfer over from the superbill to integrated billing systems for the time being.

  4. "Add Another" Diagnosis and Medication

    When documenting in a patient's chart, I can click on the "Add Another" button when I'm done adding an allergy. I would like this functionality for Diagnoses and Medications as well.

  5. Sort Frequent Diagnosis Codes Differently

    When searching for a frequent dx code I have to scroll through every one because it is currently sorting by the code rather than the descriptor. I would like this to be alphabetical. It serves no use currently as it takes less time to search for it in the search box than scroll down through the list.

  6. One click to add diagnosis to encounter

    Instead of clicking on a diagnosis within an encounter and then checking the box "add to encounter," there should be a one-click option (i.e. check box next to diagnosis) in encounter flow to attach diagnosis.

  7. Mark Diagnosis Codes as neither "Acute" nor "Chronic"

    We add diagnosis codes that were given to use from the referring doctor so we usually don't know if the problem is chronic or acute. At the moment, there is not a general section where we can enter a Dx code without categorizing it under either chronic or acute. Please add an ability for us to mark a diagnosis as neither of these selections.

  8. Sort Diagnoses by Code in Patient Chart

    Some patients have a long list of diagnoses. They are not in any particular order nor are sortable by code. It would help us to be able to customize how diagnoses are sorted, so the same dx is not entered multiple times in the same list.

  9. When only one ICD10 option exists, auto-select that diagnosis

    When only one ICD10 diagnosis option is available to be selected to convert from ICD9, the EHR should auto-select the ICD10 without requiring us to also click the Save button.

  10. Reorder or hide diagnosis encounter comments

    The encounter comments under ASSESSMENT are showing OLDEST comment first. The chart prints this and it looks like what happened in the progress note today is what happened years ago. For example, my nurse gave a Zostavax yesterday because under V70.0, physical exam, the comment was Zostavax. But the comment was from 2012. The hidden comment from yesterday was "Prevnar."

  11. Diagnosis Charting Improvements

    Associating diagnosis codes to a specific date of service went from selecting bullets (in the old EHR) to individually adding each and every diagnosis (in the new EHR). Please bring back the bullets for diagnoses.

    The PF EHR now allows you to select “Add All” diagnoses when charting. This functionality now requires fewer clicks than the previous version of the EHR.

  12. Rule Out Diagnoses

    Diagnosis should contain a Rule Out section, as we often include R/O diagnoses in our assessments and diagnosis summaries, for ongoing medical problems under clinical investigation.

  13. I have entered an ICD-10 Diagnoses, however, on the summary and the encounter I am still seeing an ICD-9 code.

    The ICD 10 codes entered at 10/01/2015 in the diagnoses at the encounter and summary shows the ICD 9 with the code description. It suppose to appear with the ICD 10 instead of the ICD 9?

    Hi IT Girl,

    We are storing both ICD-9 and ICD-10 with each diagnoses. By default we are showing the ICD-9 code with the description to help with the transition. However, when looking at the summary screen you will see an option to change how you would like the diagnoses to be displayed. Next to the print icon you will see the word: Term. If you click that you will see the option to choose how you would like to see the Diagnoses displayed.

    By term displays the way that diagnoses have been displayed over the last several years, but the ICD-9 code, and the term or description that you chose when you added the diagnoses.

    By ICD-9 codes will display the ICD-9 code and description.

    By ICD-10 codes will display the ICD-10 codes and description.

    Changing this will affect the summary, encounter,…

  14. Display DSM diagnoses in Diagnosis Search

    I'd like to be able to search the ICD code or a partial name and get both DSM results and ICD results in the list of search results. Currently I have to give a quite specific name to get DSM results to pull up, while ICD results are found much more easily.

    Example: When I search for "314" or "314." or "314.*" I don't get the DSM diagnoses that map to 314.x, I just get the ICD-9 diagnoses. If I search "attention" I get the ICD-9 diagnoses for Attention Deficit but none of the Attention Deficit diagnoses from the DSM.…

  15. Frequent Diagnosis Picklist

    There used to be a sheet on which you could check off multiple diagnoses. In PF, the diagnosis section can have a list 20-50 of the most common diagnoses of a practice that can be checked off individually or "select all" and then saved to the chart with one click.

  16. Display Diagnosis End Date in Assessment

    Please list the start date and end date in the Assessment section of the chart note. Otherwise you have to look at the diagnosis list to see if a diagnosis is ended as you cannot see it in the chart.

  17. Display diagnosis comments on screen and print

    Currently I can't see the diagnosis comments in the note. It's important to me to be able to see these at a glance.

  18. Hide Historical Diagnoses when Ordering

    Historical diagnoses are still listed when ordering labs and medications. We would like the ability to customize this feature for our practice and disable them.

  19. Customize Display for Stopped Diagnoses

    When a diagnosis is stopped, it is not clearly visible next time that diagnosis is no longer active. For example diabetes without complication(250.00) is now changed to Diabetes with renal complication(250.40) the old diagnosis is stopped. But when coders or Dr is doing next note he cannot readily see that 250.00 is discontinued. If a line goes across then the doctor and coder will avoid that diagnosis.

  20. Diagnosis properties: New, Stable, or Worsening

    I am required to state if a diagnosis is new, stable or worsening. A way to mark a diagnosis as one of these on each encounter would be helpful.
