Message a patient with an education form or instruction sheet

It would be great to be able to message a patient and attach a file such as an education form or instruction sheet. Or provide the education forms for the practice in practice fusion.

Hi there—can you describe a scenario where this would fit in your workflow, to help us understand the request a little better? Is this something that you would send to the patient usually when an initial diagnosis is entered? Or something done more ad hoc?

    I am not clear, so is there a way to send patients emails with education materials yes or no? That is the primary reason I signed them up to the portal so I could email them educations forms rather than continue printing them.

    Agree with all of the comments before me. Rating scales, medication information sheets, referral information, information about diagnoses. So many things can be sent through this portal.

    It would be very helpful to be able to send documents attached to a message or thru the patient portal. We have specialized patient education and "how to" instruction sheets that we have to email separately to the patient. It would be nice to have all communication thru the portal so it can be documented.

    It would be very helpful to be able to attach documents to emails to patients via the portal, that relate to their healthcare. Without this feature, I either have to use snail mail to send documents securely or risk emailing over an insecure connection.

    Info sheets (sleep hygiene, cognitive behavioral techniques, mindfulness practices, etc). Plus rating scales (PHQ-9, GAD-7, etc).

    Most of the time, patients ask if they can send us some of their reports or medical history and attach that through portal to us or we can attach and send that to the patient.
    I guess through referral we can attach some documents and send that to the patients instead of sending them to another physician, how ever they do not have this capability. We need some times to send the patient's patient education material and I guess now we can send some of those links to the patient's email, but as far as any other documents we can not do that.

    for surgeries, that way if they loose their pre op or post instructions we dont have to print and send them a new one, they can just access it online

    It would also be great to be able to send any of the Medline Plus or patient education articles that come with Practice Fusion directly to Patient Fusion. Even better if Practice Fusion incorporated a counter that could keep track of Meaningful Use. Keep up the great work!!!

    As a specialist, we use a lot of questionnaires and self assessments for patient's to monitor mood and anxiety. I like the idea of sending the pdf to the patient so that they can work on the form before they return... If they could send it back electronically you would really have an awesome tool for self-assessment and maintaining wellness for patients.

    I would like to use this after labs or tests come back, for instance, information on diet and exercise or a list of specialists to see.

    I'd like to add the patient info link to patient fusion. That way I can say to my patient "You can create a patient fusion account and there you will find your med list with links to patient information for each medication you are taking." This would be very helpful for the patient and also be legally helpful for the provider. We can say we gave patient access to med info forms without addition work or printing them out. I don't have time to print out a 3-5 page info form or email people with links. This would be a quick easy fix. Just add the link.

    I currently finish my plan with defining exercises that the patient should be doing. I attach them to a regular email as this has virtually eliminated me having to reprint them for patients "who lost them" at there next appointment. I often attach three to four PDFs to one email. Doing this within the secure email would be so much better.

    Another great reason to have this feature available is that it gives the doctors a record of what they shared with the patient. Advice or instructions are supposed to be documented, in case the patient later says "You didn't give me that info, or I didn't know how to use the med or cold pack" for example.

    definitely! or when their cholesterol comes back high, I can send them a message and attach our diet sheet for example!

    Right now I print the pages out and hand them to the patient. But if I send them to patient fusion 1) they can't lose it 2) drives them to use Patient Fusion (right now the minority of patient's who are signed up do log in) and 3) less messing with printers on every desk. I think this is a great idea.

    This is a great idea. I can't speak for the original poster but our practice would possibly send the patient a Diabetic diet handout that we forgot to give, our back exercise handout.
