Add a risk score assigned to a patient on his chart. The care team will assign this score and should have ability to run report

Risk stratification of the patient is an important activity for the care team. The care team should be able to assign a risk score to the patient. This score should appear on the patient chart. Additionally the care team should be able to generate a patient list based on risk score. This will help in care management and give the ability to the care team to monitor the patient to up/down adjust the care.

Hi everyone – On 6/08/2017, we released the ability to add Patient Risk Score to the patient chart via the patient Summary page. We will be keeping this feature request in progress for the time being as the requested additional reporting functionality for patient risk score is not currently available.

    Risk stratification are one of the requirements.
    It should be in structured data to enable reporting with other criteria
    97% of our patients must be risk stratified.

    IN addition , we must also have a feature to report longitudinal and episodic encounters

    When will the HCC RAF risk point scores that determine Medicare reimbursement rates by added to the diagnosis choices and visible on the Diagnosis list? Choosing among similar codes to get the highest point value has become very important and PF isn't helping us with this. It's just another number to add next to the ICD code in the menus.

    Yes, Clinical staff and/or the physician to whom the patient has seen should have the ability to change/create the risk score. The front office or the scheduling should be able to run report, view etc.

    The objective is to manage the patient well, a patient with a higher score will need more care vs. patient with a lower score will need only say yearly physicals.

    PF can implement this in phases.

    In the first phase

    1. Clinical staff / Physician to enter and record the risk score in a field on the chart based on their own assessment. It can be numeric or alphanumeric.
    2. Sortable
    3. Groupable
    4. Can be specified in report Query ( for filtering etc), the report shows the last encounter date for the patient and the disease condition etc...

    In the second phase, PF can algorithmically determine the risk score. There are several algorithms available in the public domain to determine the risk score, however the physician should have the ability to override and record.
