Additional Encounter Types
Most surgeons will see patients in multiple hospitals in the ER on the floor or in the Operating room. When need to have a way to capture and add the documentation to our EMR both for billing and for documentation purposes. Currently the encounter type is limited to office.
Can you please provide additional encounter types: ER consultation, operating room procedure
Great news! Users can now add encounter types according to their SNOMED code, or HCPCS code.

I am seeing my patients at their homes, or at assisted livings, group homes... Per Medicare they all have their distinct E&M codes. I would like to see these "Encounter Types" listed separately.

Yes!!! Our administrators use the chart note type to help manage data poor specific patient populations. The only way they could figure out how to do that was to create custom types of SImple notes. They don't understand that this takes away a huge functionality of this EMR that "makes charting so easy and fast." We can't use any of the premade/shared templates. Even creating templates is tricky and time-consuming because it is assumed that the SOAP format is being used. I free text my entire SOAP note which does NOT speed up my charting!!

Some patients are not charged an EM Code. Instead the encounter is surgical in nature

I'd like to use an encounter type for a "Test Only" encounter. Otherwise all the patients coming for tests only are coming up as "Office Visits" and its killing my PQRS numbers, since I don't see the patients on those days.
When seeing a patient in a hospital, there should be a SOAP category for both ENCOUNTER TYPE & Office Visit NOTE TYPE. Can you please add Hospital to the available options? Thank you.

Please add Encounter Types OR, ER, HOME, OTHER. These are pts in schedule but the encounter reads "office visit" and this is confusing.

Yes, we need at least an "other" catergory for out of office encounters. Myself, the bulk of my gastroenterology procedural practice is at Ambulatory Surgery Center.
Shouldn't Encounter Type include Hospital Visit? Hospital visits seem to be about the only encounter type missing from SOAP?

I need to record a simple note to document information discussed during peer consultations, supervisor consultations, and specialist consultations. These notes do not fall under any encounter type with the client.
Please add an Encounter Type > Other or allow us to make custom encounter types. Thanks!

We keep track of patient's who miss their appointments. It would be nice to have "No Show" or "DNKA" as an encounter type. By doing this, you see that they missed an appointment when you review their last office visit.

We need a surgery/hospital/consult/other encounter type. We need to be able to choose this when scheduling. My concern with the current system is when we put these things on the schedule with the available choices they become part of the denominator for Meaningful Use. Can anyone suggest another work-around in the meantime (this issue has been out there for over a year) to keep these encounter types out of the denominator.
For record keeping in PF, Encounter Type needs Hospital Visit added & Note Type needs Hospital Visit added.
please add "note to chart" as an encounter type, or allow custom name for encounter types
I want a template that does not include the required“Details", "Description", “Subjective", “Objective", “Assessment", and “Plan” headers. Pls, I have always used and E-SOAP format which is not an option in PracticeFusion.
Our office does not use the standard "SOAP" template created by Practice Fusion because of the flow of our office. Instead, we use a group of simple notes that we created to fulfill our needs. However, because we don't use the standard "SOAP" templates, we are unable to attach diagnosis to the encounter--we have to manually add them in to the note AFTER we add them into the summary. It would be greatly appreciated if we were able to connect the summary diagnosis with OUR notes without having to manually type them in.

1. "New Encounters (SOAP)" allows multiple note types (note to chart, record review, summary etc).
2. Once a note type is chosen, however, "encounter type" choices are limited - most important, there is no option for "note to chart" or "records review" or "reminder for next visit" etc.
3. Please allow us to add "encounter types" or at least add "Note to Chart" as an option.
This seems like such an intuitive customization! When I look at clients' timelines, it would be great to identify quickly which services they have received (i.e., individual therapy, group therapy, activity therapy, Equine-Assisted Therapy.)
