Track Ongoing Treatment Plan

For mental health, we have a patient-specific treatment plan that should be accessible to case notes for that client and treatment plan reviews.

    Treatment plans and treatment plan reviews also need to be signed by client's and clinician

    The ability to have separate cases or tracking would be very beneficial.

    This would be helpful for physical therapy and chiropractic services also.

    A treatment plan template would be helpful, but it should not be in SOAP note format. What is the status of the plan for adding this type of template?

    A treatment plan would ideally be in its own section of the chart, with prompts for the clinician to check whether goals have been met.

    For example it might look like:

    Goal 1: By 12/24/15 Pt will have a 2 month period in which no work is missed as a result of mental health symptoms.
    Goal 2: By 8/14/15 PHQ9 will show no suicidality.

    There would be some kind of reminder at each appointment for the provider to see whether the goals have been met (and therefore need to be changed to new goals), and there would be another reminder at the first appointment following 8/14/15 that Goal 2 has expired and must be updated.
