This is very cumbersome part of the charting process. The ros is critical in terms of reimbursement, liability, billing etc. Ideally a check list would work best. For template items, you would check off back pain instead of either trying to tab or finger tap ??
The ?? item is a good idea but doesn't work well for mobility (ipads/surface etc) and doesn't work well on desktop (tab key is unreliable).

a checkoff list that the patient could enter themselves would also be great.

Agreed - if we are to use Practice Fusion to easily document level of service on a superbill, then a dedicated review of systems section is essential.
Please include this feature in the next update!!

I would like the line items to be customizable from a template with:
    1. SECTION HEADERS
    2. Question lists for each section

The QUESTIONS should be presented with Yes/No radio buttons.

Several output options should be offered.

    I prefer:

    SECTION HEADER:
    Reports: ...
    Denies: ...

Even better would be if prior answers were carried forward in the radio button settings and the output read:

    SECTION HEADER:
    New: ...
    Continues to report:
    No longer reports:
    Denies: ...
