2018 Medicaid Meaningful Use Stage 2 Objective 3: CPOE for Medication, Lab, and Radiology Orders

Overview

The Modified Stage 2 CPOE objective includes three measures and the threshold requirements of all three measures must be met in order to achieve this measure. Computerized physician order entry (CPOE) is the entry of the order into the EHR and does not relate to how that order is filled or otherwise carried out. Stage 2 Objective 3 requires that you use CPOE to record the following items during your EHR reporting period:

  • Measure 1: More than 60% of medication orders
  • Measure 2: More than 30% of lab orders
  • Measure 3: More than 30% of radiology/imaging orders

CPOE for Medication Orders: Practice Fusion Meaningful Use Dashboard Calculation

  • Denominator: The number of prescriptions recorded, printed or e-prescribed (including refills) during the EHR reporting period.
  • Numerator: The number of prescriptions in the denominator that are recorded using CPOE.
  • Exclusion for Measure 1 (learn more) Any provider who writes fewer than 100 medication orders during the EHR reporting period is excluded from the corresponding measure.

CPOE for Lab Orders: Practice Fusion Meaningful Use Dashboard Calculation

  • Denominator: The number of lab tests ordered during the EHR reporting period.
  • Numerator: The number of lab tests from the denominator recorded using CPOE.
  • Exclusion for Measure 2 (learn more) Any provider who writes fewer than 100 laboratory orders during the EHR reporting period is excluded from the corresponding measure.

CPOE for Radiology/Imaging Orders: Practice Fusion Meaningful Use Dashboard Calculation

  • Denominator: The number of radiology tests ordered during the EHR reporting period.
  • Numerator: The number of orders in the denominator that are recorded using CPOE.
  • Exclusion (learn more) Any Providers who writes fewer than 100 radiology/imaging orders during the EHR reporting period is excluded from the corresponding measure.

CPOE for Medication Orders Measure Requirements: Practice Fusion Suggested Workflow

Practice Fusion suggests the following workflow to ensure that you meet the requirements of this measure within the Practice Fusion EHR.

1. Scroll to the Plan section of the encounter and click Record to add a medication to a patient’s encounter and send an e-prescription (see Graphic 1 below).

2. From the Medications tab, search for and select a medication. Click on the newly added medication and click Done to proceed with ordering (see Graphic 2 below).

3. Add directions and a start date along with any notes to the pharmacy, etc. in the details window, then click Order (see Graphic 3 below).

4. Review your prescription summary and select a pharmacy to fill the order if applicable. Click Send eRx, Print Rx, or Record Rx (see Graphic 4 below).

CPOE for Lab Orders Measure Requirements: Practice Fusion Suggested Workflow

1. Click the Actions button in the upper-right corner and select Add Lab Order (see Graphic 5 below).

2. From within the Add Lab Order screen (see Graphic 6 below), select the lab you want to send orders to and enter in the required information. If you are not connected with a lab, you have the option to select Other. Note: When entering a lab test, you must add a 'Test Code' if you are ordering a custom test.

3. If you have your office staff record orders on your behalf, they must select the EP's name from the Ordering Provider drop-down menu in the order for you to receive credit on the Practice Fusion Meaningful Use Dashboard (see Graphic 7 below). If you are connected with a lab, click Send to submit the order electronically to the lab. If you are not connected with a lab, click Print.

CPOE for Radiology/Imaging Orders Measure Requirements: Practice Fusion Suggested Workflow

2. From within the Add Imaging Order screen, select the imaging center you want to send orders to and then enter in the required information. If you are not connected with an imaging center, you have the option to select Other (see Graphic 9 below). Note: When entering an imaging test, you must add a 'Test Code' if you are ordering a custom test.

3.     • A: CPOE is a provider's use of computer assistance to enter and record medical orders (including medication orders, laboratory tests, and radiology tests) from a computer or mobile device.

  • Q: Does the physician have to enter the information for CPOE?

    • A: Any licensed healthcare professionals and credentialed medical assistants, can enter orders into the medical record for the purpose of including the order in the numerator for the objective of CPOE if they can originate the order per state, local and professional guidelines. Credentialing for a medical assistant must come from an organization other than the organization employing the medical assistant. To ensure that the provider gets credit for orders entered by other staff, the staff must select the provider’s name from the “Ordering Physician” drop-down menu within each order.
