2018 Medicaid Meaningful Use Stage 2 Objective 8: Patient Electronic Access

Overview

This Stage 2 objective includes two sub-measures, one of which requires action by the provider and one of which requires action from your patients. Encourage your patients to sign up for an email account using an email account service such as Gmail or Yahoo prior to their first appointment to help speed up the PHR enrollment process.

Please note that both sub-measure thresholds must be met in order to achieve this measure:

  • Measure 1: More than 50% of all unique patients seen by the Eligible Professional (EP) during the EHR reporting period are provided timely access to view online, download, and transmit to a third party their health information subject to the EP's discretion to withhold certain information.

    • Measure 1 Exclusion: Any provider who neither orders nor creates any of the information listed for inclusion as part of the measures except for “Patient Name” and “Provider’s name and office contact information.”

  • Measure 2: More than 5% of unique patients seen by the EP during the EHR reporting period (or the patient's authorized representatives) view, download or transmit to a third party their health information during the EHR reporting period.

    • Measure 2 Exclusion: Any provider who:

      • Neither orders nor creates any of the information listed for inclusion as part of the measures except for “Patient Name” and “Provider’s name and office contact information;” or

      • Conducts 50 percent or more of his or her patient encounters in a county that does not have 50 percent or more of its housing units with 4Mbps broadband availability according to the latest information available from the FCC on the first day of the EHR reporting period.

  • Denominator: The number of unique patients seen during the reporting period.
  • Numerator:  The number of patients in the denominator who have access to view online, download and transmit their health information within 4 business days after the information is available to the EP.

  • Denominator: Number of unique patients seen during the reporting period.
  • Numerator: The number of patients in the denominator (or patient-authorized representative) who view, download, or transmit to a third party their health information.

Note: The numerator and denominator must be reported, and the numerator must be equal to or greater than 1.

Denominator Requirements

The Practice Fusion Meaningful Use Dashboard defines “unique patient” as a patient with a signed note with a date of service that is during your EHR reporting period with any of the following encounter types:

  • Office Visit
  • Home Visit
  • Nursing Home Visit
  • Telemedicine Visit

Note: Only the provider who signs the note receives denominator credit.

Numerator Requirements: Practice Fusion Suggested Workflow for Measure 1: Patient Electronic Access

Practice Fusion suggests the following workflow to ensure that you meet the requirements of this measure within the Practice Fusion EHR.

1. You can use the auto-invite feature to provide patients timely access to their Patient Portal. Learn how to set up the auto-invite feature. In order to effectively use the auto-invite feature for this measure, you must sign your chart notes within 4 business days of the chart note’s date of service.

2. To manually enroll your patients in the Patient Portal, click the Invite to Patient Portal link in the patient’s chart header (see Graphic 1 below).

3.   Enter the patient's email address and phone number, then click Invite to patient portal (see Graphic 2 below). You may also invite a family member, caregiver, or guardian to access the patient's health record by clicking Add Users.

4.   You'll be prompted to confirm the invitation and review the information, then click Invite (see Graphic 3 below).

Anyone in your practice who is enabled to manage Patient Portal access can enroll patients in the Patient Portal and you will still achieve credit for this measure.

Measure Requirements: Practice Fusion Suggested Workflow for Measure 2: Patients View, Download, or Transmit their Health Information Online

Practice Fusion suggests the following workflow to ensure that you meet the requirements of this measure within the Practice Fusion EHR.

  1. After you enroll patients in the Patient Portal, encourage your patients to complete enrollment as soon as they get home from the appointment using the PIN code that you give them during the visit or the phone number you have for them on file. 
  2. Your patients must log into the Patient Portal at least once during the reporting period in order for you to get credit for this measure.

    • A: The following information is required to be available online to the patient unless the information is not available in certified EHR technology (CEHRT), is restricted from disclosure due to any federal, state or local law regarding the privacy of a person’s health information, including variations due to the age of the patient, or the provider believes that substantial harm may arise from disclosing particular health information in this manner:

  • Patient name
  • Provider's name and office contact information
  • Current and past problem list,
  • Procedures
  • Laboratory test results
  • Current medication list and medication history
  • Current medication allergy list and medication allergy history
  • Vital signs (height, weight, blood pressure, BMI, growth charts)
  • Smoking status
  • Demographic information (preferred language, sex, race, ethnicity, date of birth)
  • Care plan field(s), including goals and instructions
  • Any known care team members including the primary care provider (PCP) of record

More information

  • If a patient has multiple encounters notes from during the selected reporting period, numerator credit can be achieved provided the patient is invited to enroll in their patient portal within 4 business days of any one of those encounters. Furthermore, if a patient has been granted access prior to their first encounter of the reporting period, they will be added to the numerator once that first encounter has been signed.
