What information is shared in the patient portal (PHR)?

Only structured data from the patient chart (lab results, diagnoses, medications, immunizations, allergies) and certain fields of a chart note will appear. The written SOAP content of chart notes will not be included. 

  • Lab tests and results will show tests you have shared with the patient broken out by component, including their results and standard range. From your EHR lab results, you can share individual components or the full test results. Lab results will only appear if they are in final status, even if you have already shared them, and certain format of labs (namely PDF results) will not appear in full.

  • Diagnoses include specific diagnoses you have added to the Dx list in the patient’s chart or have added to a chart note by clicking Add diagnosis.

  • Medications include current and past medications you have added to the the Rx List in the patient’s chart or have added to a chart note by clicking Add medication. Medications you have prescribed will include your SIG directions for the patient in addition to other information about quantity of the prescription and number of refills available.

  • Immunizations includes all immunizations recorded for the patient from the Immunizations section of the patients chart. This includes the status of the immunization, the source and the date.

  • Allergies includes all allergies recorded for the patient from the Allergies section of the patient’s chart. This includes severity, reaction and onset.

  • Procedures includes certain screenings, interventions or assessments added in the Screenings/Interventions/Assessments section of a chart note, in addition to items selected from Quality of Care. Care plans recorded in an encounter will also appear. This information will be available to patients after your note has been signed.



  • Your patient will also see a record of any past or upcoming appointments they have, as well as the last vitals recorded in a signed chart note.
