Description:

Percentage of patients 65 years of age and older who have ever received a pneumococcal vaccine.

  • Numerator: Patients who have ever received a pneumococcal vaccination.
  • Denominator: Patients 65 years of age and older with an eligible visit (defined as chart notes with one of the following encounter types: Office Visit, Nursing Home Visit, Nurse Visit or Home Visit) during the measurement period.

  1. For all visits with patients age 65 years and older, determine whether they have received a pneumococcal vaccine prior to the encounter.

  2. If appropriate, administer the vaccine to the patient.

  3. Record the pneumococcal vaccination in the Immunizations section of the patient chart.

  • From the patient encounter, click the “Go to…” menu at the top of the page and select Immunizations. Select “Add Vaccine” and then select “Administered.” Search for the administered vaccine in the Vaccine field. The pneumococcal vaccines that meet the numerator requirements are:

    • Prevnar 13/Pneumococcal conjugate, 13 valent (CVX 133)

    • Pneumovax 23/Pneumococcal, 23-valent, adult (CVX 33)

  • Once selected, complete the required details for the vaccine and save to the patient’s chart.

OR

  • In the Screenings/Interventions/Assessments section of the encounter, search for one of the following applicable terms and select the item:

    • Pneumococcal vaccination (procedure) (SNOMED CT 12866006)

    • Booster pneumococcal vaccination (procedure) (SNOMED CT 394678003)

  • Once selected, record and save the following information about the vaccination in the modal window that opens:

    • Status: Performed

    • Start Date: Date the vaccine was administered

Historical
If administered by another provider or in another care setting.

  • From the patient encounter, click the “Go to…” menu at the top of the page and select Immunizations. Select “Add Vaccine” and then select “Historical.” Search for the vaccine in the Vaccine field. The pneumococcal vaccines that meet the numerator requirements are:

    • Prevnar 13/Pneumococcal conjugate, 13 valent (CVX 133)

    • Pneumovax 23/Pneumococcal, 23-valent, adult (CVX 33)

  • Once selected, complete the required details for the historical vaccine and save to the patient’s chart.

OR

  • In the Screenings/Interventions/Assessments section of the encounter, search for the following applicable term and select the item:

    • Pneumococcal vaccination given (finding) (SNOMED CT 310578008)

  • Once selected, record and save the following information about the vaccination in the modal window that opens:

    • Status: Performed

    • Start Date: Date the vaccine was administered

  • This 2016 measure version has been updated from the previous 2015 version specifications, incorporating the following change:

    • Removed the guidance statement as it no longer aligns with the clinical guidelines.

    • Updated the measure name to harmonize with the endorsed measure name.
