How do I sign a chart note?
Within the patient's chart, locate the chart note within the patient's Timeline. After selecting the chart note, click the Sign button in the top right-hand corner.
Note: If you do not see the Sign button, please verify that you have an edit level of Phys./MD/DO.
Can I edit/delete the chart note once it has been signed?
Once a chart note is signed, it cannot be edited, deleted, or modified. However, you can add an amendment/addendum to the note, by clicking the Add addendum button in the top-right corner of the encounter. Note: This button will only appear once the note has been signed.
Can I co-sign a chart note?
While there is not a designated feature to co-sign a note, the best way to document a co-signed chart note would be to use the Add an amendment/addendum feature after the note has been signed.
Alternatively, you can select different "Seen by" and "Signed by" providers within a note, indicating the note was "Signed by" the supervising physician. Before the note has been signed, any user with Phys/MD/DO edit level may adjust the "Seen by" field.
