How do I choose the right lab observation for my flowsheet?

Since observation values are based on the test and specimen, it's often easier to search by test to find the observation. However, some labs have unique names for their tests and observations which can complicate finding the right item to add to your flowsheet.

Below are some tips for finding the right observation:

1. In your result report, note the name of the test or panel the result is under. Search for part of that test when adding a row to your flowsheet. For example, you may have results for a patient for CBC W Auto Differential panel in Blood.

2. In the flowsheet, search for CBC.


3. Select the closest match (in this case, the first item is the closest match).

4. You can then select the observations you'd like to add.

For some results the test name is a lot different than the observation you might be looking for, like TSH or T4 (Free T4), where the test name might be Free T4 and TSH.

Adding observations to flowsheets within lab results

To ensure that you are selecting the correct observation in your flowsheet based on the results that you are receiving from your lab, you can add observations to your flowsheets directly from a lab result.


1. Within a lab result, you will see the option to Add to flowsheet next to the test name if any of the test's observations contain a LOINC code (industry-standard format), which are required for flowsheet compatibility. This option will also display next to individual observations that contain a LOINC code.

2. Select the observation(s) you would like to add to the flowsheet. Please note that only observations that have an associated LOINC code will be available for selection.


3. Select an existing flowsheet to add the observation(s) to or create a new flowsheet. If you opt to create a new one, it will be named "New Flowsheet" and will be applied to all patients by default. You can adjust these details in your flowsheet settings after completing this workflow.

Note: If you receive a notification that certain observations could not be added to the flowsheet, please contact our Customer Service team for additional information. Click OK to navigate back to the list of available observations.

4. Once the observations have been added to a flowsheet, you can review the flowsheet for the patient or navigate back to the lab result. If you chose to create a new flowsheet, click Create flowsheet to navigate to your flowsheet settings.


Troubleshooting lab values that are not populating in flowsheets

If you have selected the correct test and observations for your flowsheet rows but the values are not populating on the patient’s flowsheets, first verify that the lab report has been attached to the patient.


If the report has been attached and the lab values are still not displaying in your flowsheet, the lab may not be sending standardized codes for the results. Most labs send results in an industry-standard format (LOINC) that can automatically be pulled into your flowsheet. If your lab is not sending results in this standard format, you may customize your flowsheets to include these lab values.


To ensure that lab values populate in all of your patients’ flowsheets, we recommend updating flowsheets from your practice’s EHR Settings.


1. Within your practice settings, select the flowsheet you want to edit from the Your practice tab. Click the  Add additional observations  link to the right of the observation that is not populating any result values.  Note: By default, all observations within your flowsheet will display ‘Add additional observations’ unless you have customized your observation following this workflow.



2. Select the lab from which you receive that particular observation result.


3. Search for and select the correct test from the list of available options for that lab.



4. Search for the observation associated with the selected test. This field will pre-populate with the name of the observation row you’ve selected in your flowsheet. If you receive the message No results found for… , you can edit the search term, as the lab may have a slightly different name for the observation.


5. A lab may have several results for the test you’ve chosen. Once you have selected the result(s) you would like to be included in the flowsheet for that observation, click Save. Then, click Save in the top-right corner of your flowsheet to save your changes.


6. You will now see that one or more results have been associated with that observation. Click on the blue hyperlink again to continue adding custom results for any additional labs that your practice receives results from. You can also click the hyperlink to remove a result.


Moving forward, Practice Fusion will automatically populate these observation values into your patient’s flowsheet.
